Transgender clinic came into inception by November 2010 primarily conceptualized by Dr. Ambujam (Prof. DVL) and Dr. S.Srikanth (Prof, DVL). The transgenders in and around Pondicherry and South Arcot attend the clinic either directly or through a Non Government organization – SAHODARAN. Transgender clinic functions on Monday and Wednesday from 10am to 12 pm every week. An average of 40-50 transgenders attend the clinic every month.

On every visit, the transgenders are screened for various sexually transmitted infections and treated for dermatological diseases. Routine screening tests HIV,HbsAg, complete blood count and other relevant investigations are done free of cost. All medicines are issued free of cost.

They are referred to other specialty departments if they present with complaints pertaining to systemic illnesses where they are examined and treated free of cost. ICTC helps in counseling and assessment of their knowledge about STD/HIV and provides awareness about modes of transmission of STD/HIV and safe sex practices. Condoms are issued free of cost.

Department of DVL offers free laser hair removal treatment for all transgenders seeking treatment for cosmetic concerns. Total of 33 sex reassignment surgeries has been conducted.

  • A Team from Chattisgarh Govt. State Aids Control Society visited MGMCRI in June 2016 regarding sex reassignment surgery and other transgender activities offered which was attended by Dr.S.Krishnan(Prof and HOD, DVL) and Dr. Raji (AP,DVL).

Gender Reassignment Surgery 

INTRODUCTION

The Individuals who were born either as a Male or a Female by birth, who experience severe Gender dysphoria at their adolescent age, later in life wish to undergo Sex Reassignment Surgery (SRS) or Gender Reassignment Surgery (GRS) .

Transgenders (TGs) from all over India, visit our Hospital to undergo Gender Reassignment Surgery - Male to Female ( GRS MtF ), at a cost effective manner with excellent outcome and post operative patient satisfaction.

TGs initially visit the TGs clinic, where they undergo General Physical Examination, then, Psychiatric assessment, which involves counseling of TGs and assessment of their mental status. Next they visit Department of General Surgery after Psychiatric clearance, where they undergo investigation & preparation for surgery. This is followed by GRS MtF.

Post operatively TGs are satisfied with the procedure. TGs have also requested for Breast Augmentation procedures as a part of GRS MtF and we are working for a cost effective solution. The cost for Perineal reconstruction in our Hospital is approximately Rs.15000.

We had signed a MOU with Department of Social Welfare, Governmentof Chattisgarh for performing GRS for their Transgenders at a nominal cost.

Overall feedback obtained from TGs who have undergone GRS MtF in ourHospital : “ We have always had the discomfort of living physically as a Male though we have the mental makeup of a Female. After undergoing GRS MtF, we are feeling complete, we experience a change in our day to day activities, we can concentrate on our work, we have been given recognition as a complete Transgender as we can now accept our bodily appearance.”

ABOUT GENDER REASSIGNMENT SURGERY

Sex Reassignment Surgery (SRS) also known as Gender Reassignment Surgery (GRS) is a term for the surgical procedures by which a person's physical appearance and function of their existing sexual characteristics are altered to resemble that of the other sex. It is part of a treatment for gender identity disorder or now better termed as Gender Dysphoria in transgender people. It may also be performed on intersex people, often in infancy.

“It must be very clear that the outcome of any GRS reconstructive surgery is completely irreversible, can never be as realistic as natural ones, and after GRS, one can never become a genetic father or mother.”

Male to female (Trans Female):

The procedures involved systematic emasculation, Creation of neovagina using penile skin, creation of labia majora & labia minora by scrotal skin and creation of clitoris by debulked sensate flap of glans penis. Breast augmentation using Silicone cohesive gel or expander implant completes the procedure. Though the surgery can be performed in single stage, few further stages may be required to fine-tune the result. In addition other ancillary procedures like Laser hair removal, vocal cord surgery to change the voice etc. also can be done as per the request of the individual.

The postoperative outcome of vaginoplasty is variable. It usually allows coitus (sexual intercourse), although sensation might not always be present. The physical factors, the narrowness of the male pelvis can reduce the available area to use for vaginoplasty and limit vaginal dimensions.

Female to male (Trans male)

Female to Male is more complex and complicated procedure than Male to Female. The surgery is always staged and are : Bilateral simple Mastectomy, salpingo-oophorectomy, hysterectomy & excision of vagina (in Single or two stage).

Reconstruction of neo penis using microsurgical technique. Placement of penile implants for penile erection.

A gap of 46 month is required in between each stage for proper healing of the areas. Few further stages may be required to fine tune the results or to perform ancillary procedures like vocal cord surgery to change the voice etc.

Requirements for Sex Reassignment Procedures: The minimum requirements to start Hormonal therapy or Gender Reassignment surgery/Breast augmentation are
  • Patient must be a major(Completed 18 years of age).
  • Must be in stable frame of mind, and are living in society with the new desired external appearance (Real Life Experience) for at least 1 year.
  • Consultation & counseling with psychiatrist (mental health physician) for at least 3 months, to rule out any abnormal mental status or infatuation or temporary attraction/any secondary gain; and approval of psychiatrist to undergo hormonal therapy and/or breast surgery and or GRS.
  • Affidavit (notarized) from patient that he/she is a major and can fully understand pros/cons & take the decision regarding the therapy/surgery and understanding the irreversible nature of the procedures.
