The Department of Anesthesiology & Critical Care at Mahatma Gandhi Medical College and Research Institute was set up in the year 2001 with 3 operation theatre and Intensive care Unit. Since then it has grown rapidly and made advances by leaps and bounds. Presently the department has 10 operation theatres and a 7 bedded Intensive Care Unit, equipped with modem equipments and facilities. The department caters to basic surgical disciplines like General Surgery, Orthopedics, Obstetrics & Gynecology, ENT & Ophthalmology, and specialties like Plastic & Micro vascular Surgery, Urology, Neuro-Surgery, Pediatric Surgery, Cardiothoracic & Vascular Surgery. The fully functional cardiac cathlab is supported adequately by the department as and when required. The variety of cases integrated with ongoing clinical research & education results in wide ranging expertise in the anesthetic management of patients' of all ages. Pain clinic has been set up for Management of acute and chronic pain and per-operative care.

The department provides twenty four hours emergency services every day. The services of a consultant in anesthesia are available day and night in order to ensure the well being of patients coming to the hospital. The Emergency services are provided in Casualty, Emergency Ward, Intensive Care Unit and for emergency surgical procedures. Regular Teaching programs are being conducted by the department for undergraduate, postgraduates. Special training on Basic Life Support System / Cardio Pulmonary Resuscitation are being given to Nursing Staff, Paramedical staff, students and Common man.

The department has conducted various conference, C.M.E.'s and workshop in the field of anesthesia and Critical care. Presently various research projects are run by Faculty members of the department. The faculty members have participated in various national and international conferences.

Department has started M.D. courses from 2008 academic year. Separate pain palliative care unit and trauma centre are going to be started.

 

 


State of the Art Anaesthesia machines with automatic record keeping facilities available in every operation theatre. These machines are equipped with latest monitors which allow use of extremely low fresh gas flows resulting in extreme economy and negligible theatre pollution. It also allows us to know the exact amount of anaesthetic being administered to the patient.

Anaesthetic gas monitors continuously display the levels of oxygen, carbon dioxide, nitrous oxide, anaesthetic gases being administered. LMA classic, proseal LMA, AMBU LMA of all sizes are available which allows administration of anesthesia without intubating the patient. Latest equipments for airway management like Intubating LMA, intubating Fibro Optic Bronchoscope and Retro molar scope are also available. Neuromuscular Monitor and nerve locator helps to precisely titrate NMB for GA and accurate localization of nerve plexuses for RA.

Recently a portable Ultrasound machine (Sonosite) has been added into the armamentarium of the departmental equipments. It is widely used for nerve blocks (upper & lower limb), central venous cannulation, assessment of intravascular fluid status, bedside cardiac assessment in ICU and OR.

MuItiparameter vital sign monitors are available with each anesthesia machine for continuous monitoring of ECG, Blood Pressure (Invasive & non-invasive), Oxygen saturation, End tidal carbon dioxide, Temperature and Airway pressures.

Newer drugs for predictable anesthetic response are available like Sevoflurane, Fentanyl, Propofol, rocuronium, clonidine, dexmedetomidine etc.

Intensive Care Unit is well equipped with all modern life saving facilities. Advance ICU ventilators for respiratory support of critically ill patients are available. Multi parameter vital sign monitor with facility to monitor invasive/non invasive pressures, ECG, ST segment and arrhythmia analysis, Arterial oxygen saturation and End tidal carbon dioxide (ETC02) are available. Dedicated Arterial Blood Gas Analysis equipment, Monophasic / Biphasic Defibrillator/pacemaker is available in Operation Theatres and Intensive Care Unit within the complex.

Availability of essentially required other equipment including i.e. Syringe pumps, infusion pumps, Blood gas and electrolyte machine, intubating laryngoscope disposable items thus making complicated surgery safe.

LIBRARY
Apart from central library, 83 latest books on anaesthesia, critical care, pain management and various other topics are available in the department for ready reference. Five international journals and two national journals on topics related to anaesthesia and critical care are also available. The e-library section of the central library is available with 22 online anaesthesiology & critical care journals with full access.

Installation of New Equipments: Sonosite X-Porte
The department of anesthesiology is now equipped with a high-end USG machine Sonosite X-Porte which has significantly enhanced image quality. This has enabled us to perform both superficial and deeper nerve blocks with high precision. Use of this technology has increased the success rate of nerve blocks and decreased the incidence of complications. All central venous access in the department is being done ultrasound guidance. We are now planning to extend the existing 6 months FUGRA course (Fellowship in Ultrasound Guided Regional Anaesthesia) to 1 year fellowship program from March 2016. We strongly believe that this infrastructure addition will expand our understanding about the molecular dynamics of peripheral nerve blocks in future.


CLINICAL

The Department provides anaesthesia services for General Surgery, Gynaecology and Obstetrics, Orthopaedics, Ophthalmic, Maxillo-facial & Dental surgery, Oto-rhino-laryngologic, Neurosurgery, Plastic & Microvascular surgery, Urology, Cardiothoracic & Vascular surgery and Paediatric surgery. Every year more than 4000 surgeries are performed under anaesthesia.

Apart' from routine elective and emergency procedures, joint replacement, spinal instrumentation, arthroscopic and laparoscopic surgery, reconstructive plastic surgery, maxillofacial surgery, lung reduction surgery, cardiac valve replacement, CABG, OPCAB. The department is well equipped to provide anaesthesia services for all types of complicated and prolong surgical procedure. 14 bedded post operative wards with all modem facilitates available for immediate postoperative care of patients.

Emergency anaesthesia services are operational in two operating rooms 24 hours a day, 7 days a week and a consultant anaesthesiologist is available in the hospital round the clock. Seven bedded Intensive Care Unit (ICU) caters to the critically ill patients belonging to all the disciplines and is managed primarily by Department of Anaesthesiology with backup from other specialties.

Pre-anaesthetic check-up clinic is run as an Out-Patient Department (OPD) and pain clinic services for acute and chronic pain relief including post-operative pain relief are available with expertise from department of Anaesthesiology.

TEACHING AND TRAINING

Regular lecture classes are being taken by the department faculty for undergraduate students. Apart from theoretical knowledge, anaesthesia skills and procedures in operation theatres and intensive care are also taught to them during their clinical postings. A regular training programme on Basic Life Support System is being run by the department for medical students, nursing staff and paramedical staff. Cardio Pulmonary Resuscitation training is being to common men also.

The postgraduate academic activity is ongoing with their clinical posting in the respective operation theatres. A separate program is conducted in the afternoons three days a week. It consist of a seminar, journal club and interesting case presentation, one per week. Lectures by the consultants will be alternating with case presentations.

Interns have postings in the department for 15 days. During their postings training and hands on experience on per-anaesthesia checkup, I.V. cannulation, endotracheal intubation, Lumbar puncture, monitoring, CPR, I.C.V. and ventilator management, acute and chronic pain management are provided.

The faculty member of the department has published number of scientific research papers in international and national journals of repute. The faculty actively participated at various International, National/State level conferences/Workshops and delivered guest lectures / read research papers.

 


November 2010, the department hosted a one day CME & first state level ISA conference on various topics in anaesthesiology, critical care and pain. The guest lectures were conducted by faculty invited from specialty other than anaesthesiology. The CME included free paper presentation to encourage postgraduate activity.

July 2009, the department conducted a one day CME program on "Ultrasound in anaesthesiology & critical care" in association with ISA, Pondicherry branch. The topics included from basics of ultrasound to its current uses related to anaesthesiology, critical care.

September 2008: A one day workshop on "Basics of Mechanical ventilation" was organized wherein more than 120 delegates took part.

16th October, 2007: Celebrated "World Anaesthesia Day" at Lecture Hall. Dr. R. Sivrajan, Head of The Department, Minakhsi Medical College, and Kanchipuram delivered guest lecture on "Is anaesthesiologist still confined to operating suites"

August 2007: hosted south zone conference (SAZAC-PONCON) in association with ISA Pondicherry branch, more than 400 delegates and post graduate students attended the 3 day conference. Guest lecture, CMEs and workshops were conducted by eminent anaesthesiologist from the Country and abroad.

January 2007: Hosted the first session of the bimonthly CME of Indian Society of Anesthesiology, Pondicherry City Branch. Lectures on various topics in anaesthesia were delivered by faculty member and a Quiz Competition for Post-graduate student was organized.
